This isn't a costume, but it's made from the left-over material from my Anna dress and I wasn't sure what thread to post it in. Well, I made myself another dress. It was simple and easy. Just a circle skirt and then used the same pattern from my Anna dress for the bodice and sewed it to the skirt, then added wide straps and one shoulder has a bow on it. Oh, Shi! Ouch, that must've hurt! And to answer your question... I mainly use patterns, but I like to use a pattern as a base and then make adjustments and changes as I see fit. Sometimes I don't use a pattern at all, but I have drafted a few patterns here and there for simple skirts and the likes.
